Does Chocolate Really Cause Acne? A Study Removed the Sugar to Find Out

"It's not the chocolate, it's the sugar" is the standard defense whenever chocolate and acne come up. It's a reasonable-sounding argument. Researchers actually tested it directly, by removing the sugar entirely and giving people pure cocoa on its own. The results didn't go the way the sugar defense predicts.

The Study That Took Sugar Out of the Equation

In a double-blind, placebo-controlled trial, researchers gave young men with a history of acne capsules containing varying combinations of unsweetened 100% cocoa powder and a placebo powder (hydrolyzed gelatin), with some subjects receiving cocoa alone, some receiving only the placebo, and others receiving graded amounts in between — a dose-response design meant to isolate cocoa's own effect. This wasn't a long-term diet change either; it was closer to a single binge-style dose, swallowed all at once.

The results: a statistically significant increase in total acne lesion counts showed up by day 4 and remained significant through day 7, tracking with cocoa exposure. Pure cocoa, with sugar entirely out of the picture, was enough on its own to measurably worsen acne within days.

So What's Actually in Cocoa That Would Do This?

This is the genuinely interesting part, and it has nothing to do with fat or calories. A separate, carefully designed study compared pairs of foods that were identical in glycemic index (meaning they raise blood sugar at the same rate) — except one item in each pair contained cocoa powder and the other didn't. Despite having the same effect on blood sugar, the cocoa-flavored version consistently triggered a higher insulin response — 28% higher on average across all six food pairs tested, and up to 45% higher in one comparison. Since the foods were matched for sugar, fat, and fiber content, the researchers concluded that cocoa itself, independent of its sugar content, was driving the extra insulin release.

That detail connects directly to acne biology. Elevated insulin drives up a hormone called insulin-like growth factor-1 (IGF-1), and IGF-1 is one of the more well-established biological drivers of acne: it increases androgen activity, stimulates oil glands to produce more sebum, and promotes the skin cell buildup that clogs pores in the first place. Lab research on cultured skin oil-gland cells supports this pathway, finding that IGF-1 exposure increased both sebum production and inflammatory markers in those cells, leading researchers to suggest IGF-1 may be directly involved in acne development.

So the chain looks like this: cocoa → extra insulin release, independent of sugar → more IGF-1 → more oil production and inflammation → more breakouts. That's a genuinely different mechanism than "chocolate has a lot of sugar in it."

Before You Swear Off Chocolate Completely — It's Not All Bad News

This is where the story gets more interesting than a simple verdict. The same cocoa compounds implicated in the acne studies — specifically cocoa flavanols — have been separately studied for a very different skin outcome: sun protection. A randomized, double-blind trial investigated whether 12 weeks of high-flavanol chocolate consumption could change how much UV exposure skin could tolerate before burning, alongside effects on skin elasticity and hydration, compared to a low-flavanol chocolate group. Cocoa flavanols are naturally-occurring antioxidant, anti-inflammatory compounds, and this kind of photoprotective and skin-quality research is exactly why chocolate's skin story resists a simple "good" or "bad" verdict.

That's a real tension worth sitting with rather than resolving into a simple "good" or "bad": the same food may plausibly worsen acne through an insulin-driven pathway while separately being studied for antioxidant and photoprotective effects through a completely different mechanism. Diet and skin rarely reduce to a single verdict, and chocolate is a clean example of why.
